Green Prescription (GRx) is a free programme to improve the health and wellbeing of adults through physical activity/nutrition support to help individuals make sustainable, healthy behaviour changes. Through education, removal of barriers, and development of personal skills GRx aims to enhance physical, mental and social well-being.
How we support
- Individual consultation with a Healthy Lifestyles Co-ordinator and regular follow-ups for up to 6 months
- Resources and advice around healthy eating and physical activity
- Motivation and goal-setting support
- Practical cooking and food workshops
- Physical activity tasters
Who is Green Prescription for?
Green Prescription is a prevention programme prioritising referrals for inactive adults who are ready to make changes with one or more of the following:
- Pre-diabetes (priority)
- High blood pressure
- High cholesterol
- Weight concerns
- Mental illness
How can I get involved?
You can ask your GP or practice nurse or other healthcare professional to make a referral to Sport Wellington. Once referred, our Healthy Lifestyles team will contact you to arrange an individual phone or in-person consultation.
How do I refer my patient?
Sport Wellington accepts referrals across the Capital & Coast, Hutt Valley and Wairarapa DHB regions.
For medical centres
Sport Wellington is no longer accepting faxed referrals from medical centres with access to HealthLink eReferrals. For instructions on how to send an eReferral directly from your Patient Management System, please view the eReferral instructions (MedTech).
For secondary care and other providers
Sport Wellington is no longer accepting faxed referrals from hospital and community based health providers. Please request the email based referral form.
