What's new with Green Prescription?

Sport Wellington is delighted to continue delivering support services for families and individuals to achieve health and wellbeing goals throughout the 2019-20 year via our suite of Green Prescription initiatives. Here is an up-to-date breakdown of referral criteria, and how to refer.

 Child and Whanau

Referral criteria - what’s changed?

Sport Wellington is fortunate to provide support services that cover the life cycle, from pregnancy through to older age. However, we noted a gap in the age range criteria for children aged between one and three years, and their families. We have also applied learnings from programme engagement for pregnant women close to birth compared to those post-partum.  

We worked with the three District Health Boards to modify programme entry criteria to continue to provide services reflective of our priority participants needs.  


Maternal Green Prescription criteria: 

Up to twelve months support, for women who are ready to engage, and meet one of the following criteria:

  • Pregnant women diagnosed with pre-diabetes (HbA1c 41-91 as diagnosed through first antenatal bloods) or at risk of pre-diabetes
  • Pregnant Māori, Pasifika or Indian women 
  • Pregnant women aged 24 years or younger
  • Pregnant women with weight concerns
  • Māori, Pasifika or Indian women with diagnosed or pre-diabetes risk who have given birth in the last 12 months (new)

Target: 120 women across the region, with 70% of referrals identifying as Māori or Pasifika.


Pre-school Active Families criteria:

Up to twelve months support, for children aged 2 - 5 years (new - previously 3-5 years) and their whānau who are ready to engage, and meet one of the following criteria:

  • Children identified with weight concerns or whānau who are wanting to support their child grow into a healthy weight
  • Whānau who want support with their families eating and physical activity habits
  • Identify as Māori or Pasifika 

Target: 228 children and whānau across the region, with 70% of referrals identifying as Māori or Pasifika.


Active Families criteria:
Up to twelve months support, for children aged 6 - 17 years and their whānau who are ready to engage, and meet one of the following criteria:

  • Children identified with weight concerns or whānau who are wanting to support their child grow into a healthy weight
  • Whānau who want support with their families eating and physical activity habits
  • Identify as Māori or Pasifika 

Target: 250 children and whānau across the region, with 70% of referrals identifying as Māori or Pasifika.

Green Prescription criteria:
Up to six-months support, for adults aged 18 years and over who are ready to engage, and meet one of the following criteria:

  • Pre-diabetes (priority)
  • High blood pressure
  • High cholesterol
  • Weight concerns
  • Mental illness

 Target: 2,400 adults across the region.

How do we support?

Our programmes are centred around strength-based behaviour change/self-management support including physical activity, nutrition and other wellbeing interventions. We walk alongside people/whānau to support their own aspirations for positive change. We are mobile and able to visit people in their homes, in the community and/or together as groups.
